Transitional Care Center Our Transitional Care Center offers the most comprehensive post-acute services in Monroe County. Since we’re directly connected to Unity Hospital’s acute care services, our professional staff takes an interdisciplinary approach to help patients regain functional independence – and return to community living.
Specialized services include physical and occupational therapies, speech-language pathology, medical care, specialized dietary services, social work services, and pharmacy services. In addition, several specialty services are offered, including psychiatry, dentistry, and podiatry. The care team coordinates an interdisciplinary approach to care planning in addition to promoting independence and self-determination for each resident.
Our experienced nursing and rehabilitation staff is trained in advanced clinical competencies. We provide an expanded scope of service that includes:
- Joint replacement and complex fracture recovery
- Cardiac rehabilitation
- Stroke and neurological rehabilitation
- Chemotherapy administration
- Total parenteral nutrition
- Intravenous therapy
- Pain management
- Post surgical wound management
- Dialysis services
We also provide medical consultative services through several Unity Health System specialties, including:
- Cardiology
- General surgery
- Infectious disease
- Nephrology
- Oncology
- Orthopaedics
- Pain management
- Physical medicine and rehabilitation
- Pulmonary care
- Vascular surgery
A New Location: The Timothy R. McCormick Transitional Care Center
This spring, we’ll open our new Timothy R. McCormick Transitional Care Center, named after the retiring president and CEO of Unity Health System. Like the Park Ridge Living Center Wegman Family Cottages, the McCormick Transitional Care Center is designed in the neighborhood model, so that patients who need short-term care following a hospitalization can fully recover before returning to their own homes.
Our care philosophy is similar to that of the hospitality industry: each patient is our guest and chooses what time they’d like to eat, work on rehabilitation, and schedule other specialized services. Patient-directed care is the foundation of this model of culture change.
The new center offers courtyard views from each of the private rooms; each room has a private bath. Meals will be served buffet-style in the great room. Smaller lounges are located throughout the building to provide comfortable spaces for conversation and relaxation.

We'd like to thank the August Family, under the leadership of Burton August Sr., the late Charles August and his wife Jean, and Unity Health System Trustee Andrew August, for their generous offer of a matching challenge grant that was instrumental in helping to raise over $1 million for the McCormick Transitional Care Center. The August family has been a major benefactor of Unity Health System over the years, including their significant gift for the construction of the August Family Birth Place in the late 1990s.
