Dementia Care Program Unity’s Dementia Care Program offers unique and valuable resources. We are here to help family members and caregivers provide effective, quality care to their loved ones who have Alzheimer’s disease or other memory impairments.
The Dementia Care Program provides you with a team of dementia specialists, including registered nurses and social workers. This team will complete an in-home assessment, assist in care planning, education, and referrals to all the services you and your loved one need.
The one-on-one assessment includes:
- counseling and education about the disease progression
- social work and other support services
- referrals to community based services, including in-home assistance, day programs, and support groups

Designed to foster meaningful lifestyles for people with Alzheimer's disease and dementia, as well as to provide caregivers with a needed respite, Unity Adult Day Services at St. Bernard's received the 2001 Quality of Life Award from the Alzheimer's Association for being an outstanding service provider. The center provides an indoor relaxation room, a safe kitchen, and an adjacent outdoor fenced garden area. The facility allows for safe wandering with an escape prevention security system and affords individuals the freedom to pursue many activities. The specially-trained staff offers structured programming seven days a week. A weekend overnight respite service is also available upon request.
