Unity’s Alzheimer’s Dementia Outreach Program (ADOP) offers unique and valuable resources. We are here to help family members and caregivers provide effective, quality care to their loved ones who have Alzheimer’s disease or other memory impairments.

The ADOP program provides you with a team of dementia specialists, including registered nurses and social workers. This team will complete an in-home assessment, assist in care planning, education, and referrals to all the services you and your loved one need.

The one-on-one assessment includes:

• counseling and education about the disease progression
• social work and other support services
• referrals to community based services, including in-home assistance, day programs, and support groups
