Unity Offers Medication Management System

It is estimated that one in three home care patients are at risk for medication errors. One in four medication errors that occur at home result in admission to a skilled nursing facility. Those errors include taking a wrong dose, forgetting to take a prescribed medication, or not taking a prescribed drug on time. Medication errors can cause serious health problems and even death. Ensuring that medications are taken as prescribed is vital for the older adult, and a major concern for their loved ones and caregivers.
 
In response to this serious concern, Unity Health System has launched a program that provides a safe, convenient, and automated solution. First of its kind in the Rochester area, Unity’s MedSmart® medication alert system organizes, reminds, and tracks medications so they are taken properly and on time.
 
“Tracking medication use can be quite challenging, especially if you have different prescriptions that you need to take at different times,” said Sue White, manager of Unity’s Center for Aging. “This technology prevents medication errors that can lead to hospitalization. It also gives caregivers that reassurance and relief that their loved one is managing their medications appropriately.”
 
The MedSmart® technology involves an electronic dispenser that alerts patients when it is time to take their medication. It also allows a caregiver to log onto a secured web site and track if a loved is taking their medication correctly. The caregiver will know if pills are missed or being taken improperly and can follow up before a problem occurs. If a medication error has occurred, or the dispenser needs to be refilled, the caregiver is notified by phone or e-mail.   Staff set up the MedSmart® system, and provide training on its use to patients and caregivers.
