Welcome to My Unity: the Unity Patient Portal

Are you a patient at Unity Hospital? Use this online portal to access some important information about your visit, including:
  • Current medical problems, allergies and immunizations
  • Physicians and specialists seen during your Hospital visit
  • Certain lab results during your Hospital visit
  • Discharge instructions
  • Medications prescribed at discharge
  • Patient education materials
  Are you a patient at a Unity doctor's office? Use this helpful portal to:
  • Schedule or request non-urgent appointments
  • Access your health information
  • Request prescription refills
  • Ask non-urgent medical questions
  • Much more
Starting on Monday, June 30 your My Doctor's Office patient portal will have a new look. You'll find all of the same great features with new, easy-to-access buttons across the top of the screen. Call the Unity Contact Center at (585) 922-1234 with questions between 8 a.m. and 6:30 p.m., Monday through Friday.  
Need to enroll?  Please call our Contact Center at (585) 368-3000. We’ll send you an email with a link complete your online registration.

Need assistance?  If you need assistance logging into the My Hospital Visit portal, please call the helpline at 1 (877) 621-8014. If you have questions about your medical information in the portal, call the Unity Contact Center at (585) 368-3000 for assistance between 8 a.m. and 6 p.m., Monday through Friday. Or submit an email request for help to oncall@rochesterregional.org. The Contact Center will answer your message during business hours.
  Need to enroll?  To set up your My Doctor's Office account, visit your doctor's office and ask for an activation code (token). Once you have the code, click here and follow the instructions.

Need assistance?  If you registered for My Doctor’s Office and you have questions or issues, please call the My Care Help Line at (585) 922-1234 between 8 a.m. and 6:30 p.m., Monday through Friday. Or submit an email request for help to link@rochesterregional.org. The Contact Center will answer your message during business hours.
