Annual Appeal Donation Form
It's not often in our lives that we get a chance to make a real difference, become part of something meaningful that has a direct benefit to others, share a common goal, and shape the future.
Building on a 20-year history of charitable giving in Rochester and western Monroe County, St. Mary's Foundation and Park Ridge Foundation successfully united as Unity Health Foundation in August 2001. The tax-exempt, not-for-profit Unity Health Foundation develops financial support for Unity Health System's programs, services, and related community needs.
Unity Health Foundation supports the ability of Unity Health System to fulfill its mission by actively encouraging charitable support and providing responsible management of its assets.
The Foundation is governed by a 30-member volunteer board of directors comprised of individuals from the Greater Rochester community. Members are elected for up to two three-year terms. The Foundation is a member of the Rochester Grantmaker's Forum and the Greece Chamber of Commerce. An annual audit is prepared by Ernst and Young, LLP.

Today, more than ever, we need people who recognize that strong, healthy families are critical to a strong, healthy community. And, at a time when health care needs are at an all time high, gifts from individuals, businesses, and organizations will help us invest additional resources where they can bring the highest and most enduring returns now and for future generations.
