Unity Health System
Warren Hern became president and chief executive officer of Unity Health System in January 2010. He had previously served as executive vice president and chief financial officer of Unity Health System, having held the CFO position since 1985. Hern joined Park Ridge Health System, the predecessor to Unity Health System, in 1976 and progressively assumed greater responsibilities.
Hern served as national chairman of the board of directors of the Healthcare Financial Management Association (HFMA) during 1996-97 year, and prior to that served in various officer positions and on the board of directors. He is a HFMA fellow and a member of the JPMorgan Chase Rochester Region Advisory Board.
Hern holds an MBA from Rochester Institute of Technology and a BS Degree in Accounting from Southern New Hampshire University.

Putnam played a crucial role in the formation of Unity Health System. As President/CEO of St. Mary’s Hospital, he represented St. Mary’s Hospital and the Daughters of Charity National Health System in negotiations with Park Ridge Health System to create the successful affiliation creating Unity Health System. Restructuring the two health facilities involved transforming St. Mary’s Hospital from an acute care facility to a community health center that serves some of Rochester’s most vulnerable and underserved.
Putnam is executive vice president and chief operating officer of Unity Hospital. In this administrative leadership role, Mr. Putnam oversees the operations and development of services at Unity Hospital and is responsible for Unity's extensive behavioral health programs as well as Unity's employed physician group.
Putnam came to Rochester in 1984 when he accepted a position at St. Mary’s Hospital as the executive director of Westside Health Services, and served in a variety of administrative positions until he was appointed president and chief executive officer of the hospital in 1995. His accomplishments include the development of a wide range of community services in inner city Rochester, New York, for which St. Mary’s Hospital received the prestigious Foster G. McGaw Prize in 1996. Prior to his years of service as a health care leader in Rochester, Mr. Putnam was the executive director of North Jefferson Health Systems in Clayton, New York, and administrator of Big Valley Area Medical Center in Belleville, Pennsylvania.
Mr. Putnam holds a bachelor of science degree in Biological Health from Pennsylvania State University. He served in the U.S. Army as an interrogator and linguist.

 Sandra Brown MacWilliam

Sandra MacWilliam provides administrative oversight for the many services offered by Unity Health System to older adults and the chronically ill and disabled in our community. Under the leadership of Mrs. MacWilliam, Unity has expanded and enhanced its senior housing, skilled nursing facilities, home and community based services, geriatric physician office practices, and acute physical medicine and rehabilitation services. That expansion includes affordable housing complexes for seniors, programs providing care management and non-clinical services to seniors living in the community, and day care services designed for developmentally disabled adults.
MacWilliam has overseen Continuing Care Services of Unity Health System since Unity’s inception in 1997. Prior to the creation of Unity, she served as senior vice president of Continuing Care Services for Park Ridge Health System. She has worked as a health care administrator in organizations throughout New York State since 1979.
MacWilliam holds a master’s degree in Health Systems Management from Union College, and a bachelor’s degree in Biology from Southern Illinois University.

 Thomas Crilly

Tom Crilly was named senior vice president and chief financial officer in January 2010. He previously held the position of vice president and corporate controller since 1999. He joined Park Ridge Health System, the predecessor to Unity Health System, in 1991 as director of Accounting.
Crilly received his bachelor's degree in Accounting from St. Bonaventure University. He is a certified public accountant and a certified fellow in the Healthcare Financial Management Association (HFMA). Crilly has held a number of leadership positions within HFMA, and is a past president of the Rochester Regional Chapter.
Crilly also is also a board member of many local organizations, including Camp Stella Maris, Nazareth Schools, Make-a-Wish-Foundation, and Western New York Hospital Risk Management Group.
 Michael David Nazar, M.D.
Vice President of Primary Care and Community Services
Unity Health System
As vice president of Primary Care and Community Services for Unity Health System, Dr. Michael Nazar oversees the operations of Unity’s 20 family medicine, internal medicine, ob/gyn and dental practices. He also plays a key leadership role in the operations of Unity’s Community Services, a vast array of health services that include programs for the underserved and most vulnerable.
Previously, Dr. Nazar served as chairman for the Department of Family Medicine for St. Mary's Hospital from 1994 to 1998, medical director of Westside Health Services from 1987 to 1988 and practicing as an attending physician in Family Medicine at St. Mary's since 1985. He was appointed vice president of Primary Care in 1996, with the added responsibility to the Community Services programs in 2006.
Dr. Nazar received his medical degree from the University of Connecticut School of Medicine in 1982. He completed his residency in Family Medicine at the Highland Hospital, University of Rochester School of Medicine & Dentistry in 1985. He is Board Certified in Family Medicine and Geriatric Medicine and a clinical associate professor of Family Medicine at the University of Rochester School of Medicine.
Community Involvement and Board Memberships:
Dr. Nazar’s professional affiliations include the American Academy of Family Practice, American Geriatrics Society, Rochester Academy of Medicine, and the Monroe County Medical Society. Dr. Nazar has also been involved in many community committee memberships, including the Rochester Community Individual Practice Association, United Way Community Allocations committee, and the Finger Lakes Health System Agency. He currently serves on the Board of Directors for the Finger Lakes Health System Agency and is the Unity representative to the Community Technology Assessment Advisory Board (CTAAB). He is a past board member of Partners First based in St. Louis.
