Patients Experiencing Improved Glucose Levels through Unity’s Diabetes Collaborative

 Unity Health System’s Community Diabetes Collaborative (CDC) has helped Rochester-area patients with diabetes improve their blood glucose levels by 14 percent in the first 18 months.

The CDC connects providers and patients by providing opportunities for patients and health care providers to collaborate and keep patients on track. It also allows providers the ability to share data from multiple electronic medical records through the Rochester RHIO.

When the entire care team, including the primary care physician, specialists and nurse care managers have access to patient health data and test results, they can work together as a team to support the care and education of a patient. This could results in better disease management and reduced hospital and emergency room admissions.

 “Diabetes plagues many around the world—with more than 146 million cases worldwide. And our area isn’t immune to this problem with nearly one-in-10 adults in Upstate New York living with the disease,” said Scott Stratton Smith, M.D., Primary Care Provider at Unity at Chili. “We’re empowering patients to become part of their health care team. The collaborative has provided opportunities for patients and health care providers to collaborate and keep patients on track.”

Highlights of the Community Diabetes Collaborative include:

Intensive Insulin Management Program

The Intensive Insulin Management Program empowers nurse care managers to work directly with patients, under the supervision of a physician, and use a variety of tools to control blood sugar levels and improve their overall health. Since the inception of the program, more than 70 diabetes patients have reached their target blood sugar levels.

Diabetes Care Dashboards

Diabetes care dashboards identify gaps in care so that nothing "falls through the cracks" and all required tests and preventative care can be managed for the patient.  Prevention registries identify who is at risk for diabetes, so that education and other interventions can be targeted to keep these patients healthy.

MyUnity

A secure patient portal, MyUnity, allows patients to access their medial records, lab results and educational materials, as well as request appointments and discuss treatment concerns with health care providers.

Unity Diabetes Community

An enhanced Unity Diabetes Community web site unitydiabetescommunity.com invites patients to read stories from peers, engage other patients and endocrinologists and certified diabetes instructors.
