Unity Benefits Enrollment

You provide our customers with the best possible health care experience. You’re committed to your job and feel good about the work you do. But while work can bring satisfaction, it must bring more than that. That’s why at Unity we’re committed to ensuring you have access to benefits that can make a real difference for you and your loved ones.

The right combination of benefits can save you money, and help protect you and your family in good times and bad. Take the time to look at the information in these pages, and to think about your own situation and needs.

All full-time and part-time employees regularly scheduled to work at least 20 hours per week are eligible to enroll in Unity’s benefits program. You can also enroll your spouse, children, domestic partner and domestic partner’s children in many of these plans.

You’ll need a username and password to enroll. If you’ve never logged into the Benefits Enrollment website (known as “UltiPro”) before, you’ll find a username and temporary password in the packet you received from Human Resources. Once you log in, you’ll be prompted to create your own password, which must be 8-15 characters long and include at least 1 upper case letter, 1 lower case letter and 1 number.

If you’ve logged into UltiPro in the past, either during past Open Enrollment periods or on another occasion, log in using the same username and password you used before. If you’ve forgotten your log-in information, you’ll find your username in the enrollment materials you received from Human Resources. Log in with it and then click on the “forgot your password?” link. You’ll be prompted to answer three security questions.

You can enroll in or change or adjust your benefits on only three occasions:

  • As a new employee, you are eligible for coverage on the first day of the month following (or concurrent with) your hire date. You must enroll within 30 days of the date you become eligible.
  • Each fall during the annual Open Enrollment period. (Changes go into effect on January 1 of the following year.)
  • Any time during the year in the event of a change in your job status (full-time /part-time) or in your personal or family status, known as a “qualifying event.” Click here for more information on status changes and qualifying events.
